Why WOULD YOU NEED case management?

You are aninsurance/claims adjuster and need help with an injured worker. They are calling you with a lot of medical questions. They are having difficulty getting their medications. They do not seem to be progressing as expected with the current plan of treatment. You are not sure they are getting the care they need. You have a lot of questions about their medical status and treatment.

You are an injured worker. You have sustained a life-changing injury while on your job. Do you know who to see once you are discharged from the hospital? Are you aware of the reputations of the doctors in your community? Do you know who the best specialty physician is in your community to care for your injury? Do you know how to self-refer to the specialist you will need to see to ensure the best rehabilitation? Do you know anything about the multitudes of medications you have been sent home with? Who do you call for problems with getting your medications from the pharmacy?

The same questions could be asked by anyone who has fallen ill and is especially true in the elderly population. This population is very vulnerable to the effects of multiple physicians writing multiple prescriptions with little or no communicate with each other.

These are the reasons you need a Nurse Case Manager. A Nurse Case Manager is someone who is specifically trained to assist an injured or ill person with the coordination of their medical care. What does that mean? It means Nurse Case Managers assure that patients get the best possible care so that they will have the best possible outcome. Nurse Case Managers usually know how, when and where to access quality care. They are communicators; making sure that all parties involved with a patient’s care are apprised of their healthcare needs. They are advocates and educators. They work with the patient, providers, insurance carriers and employers to help the patient transition back to a productive life.

H&H Case Management has been doing just that for several years. Contact us today at +1 (832) 501-3936 with your referral!
